PAYMENT and PRIVACY

  PAYMENT:     Cash and checks are accepted for payment.
 
 
 

RATES:           Office appointments     $130 per session              

                         Home visits                   $200 per session, within 10-15 minute drive of office

INSURANCE OTHER:     You will be given a receipt with appropriate insurance code and provider information, which you may choose to submit to your insurance company. Services may be covered by your insurance. Please check your coverage carefully. Questions to ask your insurance provider could include the following.

     -  What is the coverage amount per session?
     -  What is my deductible and is it met?

     -  How many sessions per year does my plan cover?
     -  How much does my plan pay for an out-of-network provider?
     -  Is approval required from my primary care physician?

 
  CANCELLATION POLICY:     Your session time is reserved especially for you. Appointments can be rescheduled  with at least 24 hours notice. If you do not keep your appointment or if you cancel with less than 24 hours notice, there will be a missed appointment fee.
 
 
 

PRIVACY:     The relationship between a client and a psychologist is protected by the law and information cannot be disclosed without the client's written consent.

The exceptions are suspected abuse of a child, dependent adult, or elder, or threatened bodily harm to self or others. In such cases, Dr. Collier may be mandated to report the situation to the police, to a government agency, or to an intended victim. Dr. Collier will try to include the adults involved in the reporting process but this is not always possible. There are a few other exceptions which are explained in the Notice of Privacy document which Dr. Collier will provide at the first appointment.

Children also have a right to a certain degree of confidentiality. Parents have a right to general information including how therapy is going.
